To request an official transcript for an educational institution, agency, or other official use, put this request in writing. Indicate your full name, including name in school, year of graduation or years of attendance. Clearly note the address to which this official transcript is to be sent. Fee is $10.00, payable to St. Paul Church. Unofficial transcripts (so designated) can also be requested in the same manner for your personal records.
